Our Story
PeachTree Health Group was founded to fill a critical gap in brain injury services. Too often, individuals discharged from hospitals or rehabilitation centers find themselves without the long-term support they need to continue progressing. Families are left searching for solutions, unsure of how to balance independence, safety, and quality of life.
We created PeachTree Health Group to be that solution — a place where specialized care, structure, and community come together. For years, we’ve proudly served individuals and families across Delaware, offering programs that blend medical oversight, cognitive and behavioral therapy, and life-enriching day habilitation (for Assisted Living Residents).
Our Approach
Our approach is person-centered. That means care begins not with a diagnosis, but with the person — their goals, strengths, preferences, and potential. We believe in building on what individuals can do, while providing the right level of support where it’s needed most.

Our Residents
The residents of PeachTree Health Group are adults who have experienced a traumatic brain injury (TBI) or an acquired brain injury (ABI). These individuals may face challenges in social, physical, emotional, and cognitive aspects of daily life — challenges that require both compassion and specialized care.
At PeachTree, we recognize that every resident is unique. Our highly trained and caring staff provide individualized support tailored to each person’s strengths, needs, and goals.
